A Community Staff Nurse is required within Community Nursing, based within inner London Boroughs of Hammersmith and Fulham, Kensington and Chelsea & Westminster. This is a Band 5 role, working on a full-time, permanent basis.

The Community Staff Nurse will be required to provide nursing care, working under the supervision of the District Nurse Team Lead (DNTL) to patients in a variety of community settings. They will undertake community nursing duties involving delivery of nursing services and holistic assessments of patients health needs using a person centred approach. This includes providing quality nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long term care institutions. The post holder will also be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.

The ideal candidate for this role will be a Registered Nurse with current NMC registration. They will have gained recent clinical nursing experience and have had exposure to working with people with complex health needs. The ideal candidate will also have previous experience in the Nursing process, care planning and be fluent in record keeping.

The successful post holder should have knowledge of out of hospital care and the role of Community Nursing and PREP. They will have also have understanding and knowledge of the management of long term conditions, infection control and general wound care.
