A Deputy Team Leader role is required within Community Nursing, based within the Inner London Boroughs of Hammersmith & Fulham, Kensington & Chelsea and Westminster. This is a Band 6 role, working on a full-time, permanent basis.

The Deputy Team Leader will be required to provide nursing care as part of the Primary Care Team to Patients in various community settings. They will actively monitor, maintain and develop the service provided. The post holder will be responsible for managing the caseload and team, with the support of the Team Leader. They will deliver a proactive, case managed approach for patients with long term care needs.

The ideal candidate for this role must be an RGN/RN with current registration, and be willing to provide evidence of recent and continuing post-registration professional education and training. They will have experience of working with patients with complex health needs, and implementing evidence based care in practice, as well as undertaking comprehensive health needs assessments and writing care plans.

The successful post holder should have knowledge of the Principles of care for patients with long term conditions, relevant NSF’s and other national standards. They will also have knowledge of the NMC Code of Professional Conduct and application to practice.
