The States of Guernsey has an exciting opportunity for someone to join their Adult Community Services as a Learning Disability Transition Co-ordinator.

This is a key role in the Community Learning Disability Team, responsible for reviewing the care and support received by individuals from the Service.

The successful applicant will take the lead on improving the transition process for service users, including assessing, implementing and reviewing their needs, with a special focus on managing transition plans, all of which require a person-centred approach.

The Bailiwick of Guernsey is located in the English Channel and made up of five unique islands. Guernsey is the largest at 24 square miles, and boasts a combination of pristine white sand beaches and stunning cliff-top scenery.
